History-the patient is a laborer and during
his work he had a fall from 6th floor and he
directly landed on the iron rod which went through
his right upper back with an exit wound through
left temporal region in total causing two entry
wounds and two exit wounds as shown

As seen there are two wounds. In scalp
the rod had bisected through the right
cerebrum crossing tentorium and exiting
through the left temporal lobe.
Surgery :
The surgery done was left temporal
and right sub occipital cranectomy.
Unique :
Surgery is unique in its own as the
length of the foreign body is the largest
in records till date. It was 18 inches
long.
Surgery was also unique because of the
new technique of homeostasis used.
Repair of right vertebral artery was
done with saphneous vein graft and left
middle cerebral artery with superficial
temporal artery.
